California Consumer Privacy Act Request Form
Under the California Consumer Protection Act of 2018 (CCPA), California residents have the right to request that businesses: (a) disclose the categories of personal information that businesses have collected about the individual; (b) disclose the specific pieces of personal information that businesses have collected about the individual; (c) delete the individual’s personal information; or (d) not sell the individual’s personal information.
If you are a California resident (or an authorized agent acting on behalf of a California resident) and would like to make a request regarding your personal information, please complete this form and follow all instructions. Fields with an ‘*’ asterisk are required to submit the form. If you are an authorized agent completing this form, then please fill out the authorized agent fields in addition to the other fields on behalf of the consumer.
